Adoption Contract

Congratulations for your new dog!

Please ensure that you submit the Adoption Contract form provided below to rescue@jackcenter.dog and complete the payment of the adoption fee prior to the scheduled dog pick-up date. A fixed fee of 290 NZD is required for the adoption of resident dogs, which contributes to the costs associated with the dog’s living and veterinary expenses, and this can be processed via the following form.

Surrenders, support, and adoptions for non-resident dogs are conducted at no charge. We welcome optional donations and contributions.

Note: Post adoption, please remember to fill the dog registration form at your local council and update the microchip information.
